Introduction:
Before you enroll in this program, make sure you read, understand and
agree with all terms in this form. The sole purpose of this program is
to provide quality education for our youth. Feel free to share with
us your ideas toward improving the program and this enrollment
procedure.
This
enrollment procedure is provided to make your enrollment easier and more
convenient. Your enrollment form will be your final registration in this
education program. Kindly do not treat this procedure as a game or joke,
and provide all the information requested as accurately as possible.

Goal:
This education program offers Sunday classes, workshops and camps to
educate our youth about Zarathushti religion, Farsi language, and
Persian heritage.
Persian
Heritage Class:
This class is planned to teach our children their Persian
heritage with lessons in Persian history, literature, and arts of various forms.
This program might use videotape and various forms of presentation
besides interaction for teaching the subject. Program will be planned based on student age
groups and knowledge. Student parents and other adults are encouraged to participate
in this class.
Zarathushti
Religion Class: This class teaches the Zarathushti
prayers and principles required for a child to take the initiation
"Navjote or Sedreh-Pushi" ceremony. A student must enroll for
six terms, or two years to learn all the finish the course. The minimum
student age is eight when a child has basic reading skills of English
language.
Farsi
Language Class: This class
teaches the Farsi language reading and writing skills to children using
Farsi as their second language where English is the main language,
taught by Mrs. Mehrbanou Emaili. Mrs. Esmaili has taught Farsi to our
children for 19 years successfully. The minimum age is eight
when a child has basic reading and writing skills of English language.
Attendance Student
Participation:
You are encouraged to impress upon your student
the importance of cooperation in the classes. If any child shows
disruptive behavior during a class session, the teacher may ask his/her
parent to a) stay in the classroom and help their child to cooperate, or
b) cancel the child’s enrollment. The main goal of this program is to
teach the children who fully attend and cooperate with the program.
Attendance is very important in this program. If your child misses two
classes during a term, the teacher will cancel his/her enrollment.
Cancellation of enrollment requires your child to remain absent for the
remainder of the classes of the term.
Your
child will take home a simple and short homework assignment every week.
Please help your child with the assignment and sign the homework
coversheet. Should you have any questions on the assignment, feel free
to call the teacher.
Teachers
will not be responsible for children left unattended outside the
classroom or after the class session ends.

To apply for scholarship (or sponsorship), prior to sending your
enrollment form, contact Program
Director. Applicant's personal information will remain
confidential.

Sunday Class will meet at JLS middle school campus in Palo Alto, unless

Enrollment
Steps:
For

Step
1:
Read
this agreement carefully, make sure you understand and agree all the
terms.
Step
2:
Choose
the classes and make sure your student meets the requirements. For
Farsi class enrollment, choose the class you enrolled last term.
Step 3:
Fill out the form below; make sure all the information is accurate and
final.
Step 4:
Click on Submit button to send your enrollment information to Vohuman.
Step 5:
As soon as your enrollment information is verified, you will receive a
confirmation email, or phone call.
Step 6:
You must attend the parent meeting -- 10am,
April 10th, 2003 at JLS room
83. You will be asked to make your enrollment check to Vohuman at the
meeting. Missing the meeting or payment will cancel your enrollment.
Enrollment
Form (Deadline 4/10/2003):
This portion of enrollment must
be submitted by parents of children under 18.
You must fill out the fields marked by star* in order to submit your
form.
The first section of the following form is designated for parent
information and each section below needs to be filled per student.
Class Cancellation:
Each class needs (5) or more enrollments to start. In case this
requirement is cancelled, we shall notify you one day after the
enrollment deadline by phone and email.
